BOX 3.1: A SUSPICION OF ALZHEIMER’S DISEASE

Mr. X, a new patient accompanied by his wife, has come for a consultation. He appears to be in his mid-70s. He greets me courteously, gets up from his chair, walks to my office, and sits down. Nothing special about his movements, which are performed normally.

“What can I do for you?”

He turns his head toward his wife as if he is asking her to answer and says: “I don’t know.”

His wife: “Well, Doctor, he is losing his memory.”

He remains silent.

“Can you give me an example?”

“Oh, yes, several! For instance, he is always asking the same questions. If I ask him to do some shopping and give him a piece of paper with the list of what to buy, when he comes back, he’s forgotten to buy most of the food and has lost the paper. He is even forgetting the names of his grandchildren. But you know what is strange, he remembers things that happened in the past, like our wedding.”

“Is he oriented to time and place?”

“You are right, sometimes I have the impression that he does not know where he is. Anyway, he is totally lost with the dates.”

“Otherwise, his general behavior is normal?”

“Yes. He sleeps normally, he eats normally, he walks normally.”

“Mr. X, are you concerned?”

“Not really.”

“He is not, but I am! He is a person who is usually so calm and quiet, but now sometimes gets angry for no reason. The other day, he accused me of taking money from his wallet!”
